Support services for older people

AccessAbility can coordinate support services for people who have age-related needs and live in Whanganui and are over 65 years old.

Your district health board (DHB) will refer you to AccessAbility. After we receive your referral, we will contact you. We will then work with you to find and organise supports that suit you.

Support ideas could include:

    • support from whānau and friends
    • local networks
    • community organisations
    • aged-care organisations
    • home support
    • rest home care
    • other government-funded supports.

Who is eligible?

If you’ve had a needs assessment with AccessAbility, we’ll work with you to find and organise supports that suit you.

AccessAbility can coordinate services for people over age 65, who have an age-related disability, live in Whanganui and have been referred by their DHB.

Staying in touch

If your needs or circumstances change at any time, let us know so we can talk with you and review your plan.

Please tell us about any changes which might affect your disability support needs, including:

    • going to the hospital
    • significant changes in health and disability
    • changing your contact details
    • moving house.

Even if we don’t hear from you, we will make sure that you are re-assessed every three to five years. We’ll get in touch with you to arrange that meeting when it is time.

You can also ask to be re-assessed earlier, particularly if there are any big changes in your life.

Your feedback is important

We want you to live the life you imagine – so if something isn’t working for you or if it’s going really well, please let us know.

We’d like to ask you for some feedback about your experience with our Needs Assessment Service Coordination (NASC) and Local Area Coordination (LAC) as this will help us improve the services we provide.

The feedback takes approximately five minutes to complete.

If you don’t want to complete the survey you can still tell us what you think by email or phone.
