Service Coordinator – Hutt Valley (Fixed Term)

We are a national organisation offering a variety of services that help people enhance their individual well-being. This position is within our Needs Assessment and Service Coordination (NASC) team and we work with people who have physical, intellectual and/or sensory impairments who are mostly under 65 years of age.
The Service Coordinator position is a permanent and full-time role. It would be suitable for a person who has professional or personal experience of the disability sector and is empathetic, well-organised and who has great relationship building skills.
In this position you will work cross functionally within our NASC team to ensure that high quality services are provided to our clients. You will be responsible for managing supports for an existing caseload of clients of all ages, including undertaking reassessments and liaison with a range of contracted providers. You will also be meeting with new clients and working with them and their whanau to identify strengths and needs to design new packages of disability supports.
This is a busy role with competing demands. Our successful candidate will understand the importance of meeting both client needs and performance targets. They will also have:
- great people skills, with the ability to build effective relationships and work cross culturally
- solid administrative and/or case management experience with practical report writing experience
- well-developed problem-solving skills and high-levels of personal integrity
- a good understanding of the health and disability sector.
